Taiyo Yakuhin Voluntarily Recalls 10,080 Frazemicin-S Injection Kits

Japanese generic drug maker Taiyo Yakuhin voluntarily recalls 10,080 Frazemicin-S (fosfomycin) intravenous 2g kit, because the kit was mistakenly mounted with Indast (imipenem cilastatin) 0.5g kit bottles. The recall items had been distributed to 81 medical facilities nationwide from August to December. The company was alerted to the error by one medical facility who reported a color change in the liquid.
