Australia's Therapeutic Goods Administration says

adverse events involving medical devices have led to 40 deaths and serious injuries to another 870 people in the past year. The agency acknowledged "a substantial number" of such events are not reported to the device makers or the TGA, so the number could be much larger. The TGA would not reveal the identity of the devices involved or their makers, but said its report relied on mechanical and electrical malfunctions and other problems reported to the agency in the past 12 months.
