Health authorities in Vietnam's capital Hanoi have been unable to determine the reason a four-month-old child died after receiving a vaccine nearly a week earlier. The child received what was considered a routine infanrix hexa vaccination against diptheria, tetanus, whooping cough, hepatitis B, polio and a type of flu. Just hours later he died on the way to a hospital where the child's family reported him as healthy before receiving the shot.
